What makes a useful local request?
If carbon-monoxide poisoning is suspected, get outside to fresh air immediately, then call 911. Active fire or urgent danger also needs emergency services; do not wait for this routine form.

- Name the appliance and fuel if known, and explain why you need the visit.
- Share any prior report with its date and a short description of the symptoms you have seen.
- Add photos of areas you can reach without climbing or dismantling anything.
- Ask whether the work is cleaning, inspection or repair, and ask about access limits.
